Guide to completion of Application Forms.

The instructions must be read through carefully before completing the application form.
The information required in completing the application form includes:

• Fees are paid by Banker’s Pay-in slip to the institution, not by cash
• Fees are determined at the beginning of each academic year by management
• Fees must be paid before the end of the 1st week of re-opening
• The fee for the academic year is approved by the school board of governors using Ministry of Health Training Institutions’ bills as a guide.
